Dayton Fan Control Switch
Even though it looked a little different, it was a universal fit and worked perfectly

Remake
It is very shaky. Slides very easily so we had to wrap both of the legs with non slip rubber sheets to keep it from sliding on hardwood floors. Also ended up setting the brace under the top step on a rug to keep it from moving while dog was climbing up.
