Valve failed

I used this for injecting co2 in an aquarium. The valve failed and leaked the whole bottle in 3 days. I thought maybe I didn't tighten properly so I brought it to my local sporting goods store and they tried to refill it but the valve on the bottle failed. Had to purchase another tank.
