Nicholas Cage plays a paramedic working among the filth and mental desolation of New York City's Hell's Kitchen in the early 1990s. He becomes haunted by the visions of a beautiful 18 year-old girl whom he was unable to resuscitate. Soon after, he meets Mary, Patricia Arquette, a recovering drug addict when he attemps to save her Father. He becomes very confused with life as he begins to be consumed by all the death and violence. His driving partners are a bit wacko or different. Nicholas Cage tries unsuccessfully, to be fired. He life is nightmarish as he trys to redeem the lives of Rose, Mary, and himself in the process. It is a very different movie, you may have to watch a couple of times to get it but you should.
The action sequences are wonderful- real enough to watch, but still will some extra drama added for enjoyment. Keeping in mind that the movie takes place in the 1990s will help certain medical professionals from getting frustrated. The acting is superb. Cage delivers a great performance, and both Goodman and Rhames deliver great supporting performances (their should have been more scenes with Goodman). If you're looking for a movie that dwells on the action and stress that Paramedics face (both internally and externally), then you will enjoy this film. Also, this is yet another good example of why Scorsese films are worth watching.
Excellent Scorsese film, superb Nic Cage performance, and hard-to-find disc!

First off. If you aren't in EMS you won't understand most of the movie. They use alot of true terms. The film isn't all that realistic. There are some critical mistakes made but, still all in all fairly accurate. If you have ever ran Code 3 (Lights and sirens) you know what its like to avoid accidents and what its like to work on a paient and then suddenly just when you thought you won, you didn't. I see alot of myself and co- workers in Tom Sizemore, John Goodman, and Ving Rhames. They personalitys are very accurate of EMS. There is a side of EMTs that the public dosn't see and they show it well. The movie is based on a book written by a former NYC Paramedic. Read it and then watch the movie, it fills in the gaps. Lastly, I will say, watch this movie and can you imagine yourself in that poly/cotton shirt, with a scope around your neck? I have been doing this job for 3 years and this film makes me remember, "It DOES get worse." Brian EMT-P 3rd WatchRead full review
A must have for all Nick Cage fans. Fast paced action, funny, yet serious at times. Worth watching a 2nd time. With Scorsese as the director , co-stars Goodman, Sizemore & Rhames ( I'm not a Patricia Arquette fan ), you can't go wrong.
