The design of the holder (the clip part) is the fault, if you do not have a case or protector it would probably be alright. The ears of the clip are curved so it only makes contact on a small area, as with my phone the case increases the thickness of the phone, and in turn the clip just doesn't secure the phone as tight as it should, so if you hit a rough patch of road or a pothole, the phone usually pops out. The item itself is constructed well, if the clip edges were straight instead of curved I'm sure it would perform much better.
