It's label says 220vac, but works on 110vac to 125vac fine; only thing is the controller to relay output (ports 4 and 5) instead of 12vdc, is 5.8vdc; which is not a problem because the operating range of the relay is from 3vdc to 32vdc. Conclusion: It works on 120vac. See picture.

I contacted the vendor and they offered a replacement or a refund. It's difficult to change and I elected for the refund. I shipped it back with their label and they refunded my full purchase price. Kinda negative with the failure, but turned out as good as possible. Kudos to the vendor for making it right!
