As advertised, thanks! The PCB in my existing drive was giving me fits. I repaired the power cable, changed the IDE cable, reflowed all the joints and replaced all the caps, none of which changed it's behavior. I think the ROM on the board was going out. This fixed my problem!

As described (used & works) somewhat. This 20 year old controller port has an electronic board in it and there were no pictures in the listing of the rust on the metal shrouding. This could be an indicator of H2O damage which could cause it to fail over time. It was also very dirty but it's ok enough to keep. I'm giving seller positive feed because I think they are generally a good seller and they shipped item in a timely manner, it was packaged well, and they answered my questions.
