I am literally not aware that it happened any time, that the adhesive did not hold. Are you sure that you degreased everything well and that you clamped the part for a few hours before you added load? The adhesive is from 3M and chosen exactly for this material combination. If necessary I can send you a spare pad, but its crucial that it is applied correctly.
