The item arrived new in package, but the packaging was crushed right on top of the USB plug. Looking carefully again at the original listing's photo, the damage was already present before shipment. Once I opened it, I found the USB plug had been crushed so badly it had deformed the plug and made it unusable. Instead of the hassle of a return, I just took a pair of pliers to the plug and eventually bent it into a functional shape. Mouse works now.
