Le trastuzumab est utilise en situation adjuvante dans le traitement du cancer du sein surexprimant le recepteur HER2. Le but de tre etude etait d'evaluer retrospectivement la duree de traitement effective chez les patientes operees d'un cancer du sein dans tre etablissement entre janvier 2005 et decembre 2008. 96 patientes ont ete operees et traitees par trastuzumab. 95.8% ont recu une chimiotherapie per-operatoire. La duree moyenne de traitement etait de 49.66 semaines. Seules 51% ont ete traitees pendant les 52 semaines, 36.6% ont poursuivi le traitement au dela de 1 an et 12.5% ont interrompu les injections avant 1 an pour effets indesirables ou refus de continuer. Les durees de traitement superieures a un an etaient liees, a l'administration hors AMM du trastuzumab en situation pre-operatoire qui s'est ajoute a l'utilisation adjuvante. Les consequences cliniques des deviations de therapie ne sont pas connues car la duree optimale de traitement du trastuzumab reste a definir. Des essais testent actuellement des durees courtes de traitement (6 mois versus 1 an); de plus les resultats de l'etude HERA (2 ans de traitement versus 1 an) ne sont pas encore publies.
