Here's a guideline for how I visually grade records. Not all records will fit "exactly" into a certain grade, and one should assume a certain amount of tolerances within each grade. I am not an expert nor collector. Records are graded visually, unless noted that it has been tested. 

 
Vinyl Grading
Mint - Clean and glossy,  looks like it has never been played.
Nearmint - Very clean with just a few light marks or scuffs.
Nearmint / Excellent - One side is nearmint and one side is excellent
Excellent- A few light superficial scuffs or light scratches. An enjoyable listening record with possible light surface noise.
Excellent / Very Good - One side is excellent and one side is very good.
Very Good - May contains some or all; some marks, scuffs or scratches; Still an enjoyable listening album but with some surface noise, maybe some static or clicking sound.
Good - It plays, has alot of scuffing, marks and/or scratches with no skipping, plays with lots of surface noise. 
 
Cover Grading
Mint - It looks like it has never been handled; may still be in shrink.
Nearmint - Very sharp corners with no impacts or bends, like new.
Excellent - Contain one or some of the following. Small amount of writing; small split in seam; small bent corner; light shelf wear; a small tear; light ring wear; a cut corner.
Very Good - Contains some writing on cover or album label; split seams; corner wear; some shelf wear; a few tears; some ring wear; a cut corner.
Good - Contains bends, creases, heavy rings wear, alot of shelf wear, water damage.
