While not sold as an adapter, this gimmick is basically an adapter for instruments with unusual receiver configurations. The way it works is that it effectively adapts your mouthpiece to something approximately medium shank. The adapter hangs out of the receiver, not the mouthpiece from the adapter. As nothing more than an extension, you will gain roughly 3/4"...not much. You could use this as a medium shank adapter in a pinch, but I don't recommend it unless you can't afford to wait for a real one to come up. A good use for this is on instruments like my Soviet Tenorhorn. This instrument's receiver is extremely deep cut, to the point that it will completely consume the shank of a Schilke mouthpiece. Using the extension allows me to play it with other mouthpieces that wouldn't normally fit, including the Kanstul French Horn adapter. It seems like it might actually sound a little better with the extension, but I won't say that it definitely does in this case. Another use I found (and why I bought it) is that I was able to convert an instrument with a very quirky 10.5mm shank receiver to play with proper sound and intonation with more typical mouthpieces. As ridiculous as it sounds, by just making the mouthpiece hang out of the receiver (nearly an inch!), the horn plays with much better sound and intonation.
